naviHealth’s RightCare Software Implemented at Ochsner Health System to Enhance Care Transitions

The progressive hospital system will use RightCare to reduce readmissions and continue its mission to “serve, heal, lead, educate, and innovate.”
Deploying our end-to-end software gives Ochsner a quality-improvement tool that stretches across the continuum of care while providing scalability to support future growth.

NASHVILLE, Tenn.—March 31, 2016—Ochsner Health System, one of the nation’s leading non-profit, academic, multi-specialty delivery systems, will implement naviHealth’s RightCare software to help optimize care transitions, reduce avoidable readmissions, cut unnecessary length of stay, and improve the overall post-acute care experience for patients.

RightCare uses an evidence-based algorithm fed by patient-specific psychosocial information to determine risk for readmission and associated need for post-acute care. That data determines the optimal level of post-acute care and supports the placement decision. Patients are matched, referred, and transitioned to services and programs that best meet their needs.

The partnership highlights naviHealth’s commitment to enabling health systems to more effectively manage emerging payment models. During the multi-year, value-based contract, Ochsner and naviHealth will measure success in terms of enhanced patient outcomes and elimination of unnecessary administrative burdens. Ochsner will deploy the software to all in-patient and emergency departments.
